The two-way cycletrack on Huron Avenue ends abruptly at the Tip O'Neill golf course, and cars are often parked leaving insufficient space to transition in or out. The fact that the driveway has been decommissioned and relocated only exacerbates the conflict.
Please consider adding appropriate signage to indicate where legal parking begins and ends.
I also see that other junctions over the cycletrack are planned to be marked in green; please consider doing adding the same "fresh Kermit" in the transition area.

Traffic signal that should be facing Trowbridge Street is mounted at a 45 degree angle, pointing towards Angelo's Pizza at the corner.
Though the pole where this signal is mounted appears to be left of the intersection and may not be viable if truly perpendicular I'd still recommend tilting it a little to the left.

This "Trail X-ing" signpost is installed within the width of the paved path, partially obstructing the path itself.
I suggest removing the signpost and reinstalling it 1-2 feet to the west, but not to leave any stump from the existing signpost as a hazard.

88 Williams Street Chelsea, Massachusetts, 02150 - ChelseaDespite the conspicuous "Stop here on red" sign, a stop line was never painted on Williams Street last summer after the road was resurfaced. The recessed stop sign is instrumental in assisting large trucks making wide turns from Chestnut Street, as well as easing an abutter's access to a home garage.

The curb-separated cycletrack has been ravaged by utility work near Sancta Maria Hospital, and any resurfacing efforts have been subpar. Toothmarks from an excavator are visible in the asphalt, either making for a bumpy ride or forcing cyclists onto the concrete sidewalk for a smoother one.

Significant gravel and debris in the shoulder / bike lane of Huron Avenue eastbound between Fresh Pond Parkway and Aberdeen Avenue.
As there is no parking on this side of the road, it should be possible to perform a street cleaning at any point in time instead of waiting for the scheduled date in the fourth week of the month.
Please re-categorize as a street cleaning request; I believe this is only allowed internally.

I understand if these are in place to help construction vehicles mount the curb near the future bike path, but they do inhibit street cleaning, evident by the surrounding sand and debris.

There's a significant amount of debris in the cycletrack on both sides of Concord Avenue. Specifically, it's sand from past winters and pieces of asphalt with the consistency of gravel.
When the Cambridge DPW takes action, please instruct the street sweeper to mount the curb.

Starting over on a previous request. See also Issue ID 767398, which was closed and archived but not actioned, and I'm unable to reopen it.
Many city services appear to be available by appointment, so I have reason to believe the DPW can possibly get to this task and send a technician to the intersection.

With push buttons now disabled due to the pandemic response, this intersection seems to have an excessive amount of time for the pedestrian walk phase. These phases are significantly longer than the concurrent phases nearby at Huron and Aberdeen, despite Huron and Holworthy being much narrower.
15 to 25 seconds of pedestrian time should be sufficient for every 60 to 90 seconds of vehicular traffic. It's currently programmed as ~30 seconds each.
Another option: Please consider programming a flashing red and/or flashing yellow light at this intersection instead for the duration of the COVID19 pandemic.
